About this position

The Patient Solution Assistant (Nurse) is responsible for personalized patient case management, focusing on health coaching, motivational support, and education to encourage positive health behaviors.

Responsibilities

• Take responsibility for the provision of personalized patient/customer case management. This will include health coaching, motivational support, assisting to set and maintain realistic goals and disease/treatment related information/education and generally focus on encouraging patients to adopt behaviors that have a positive impact to their health.
• Document processing, e.g. Patient Enrolment and all relevant record on CRM program.
• Document checking and filling, e.g. Patient Enrolment Form.
• Communicate with patients and health care professions who involve in each Patient Solution Program.
• Coordinates with other sections, department, clients and customer to solve operational issues and concerns.

Requirements

• Bachelors degree of Nursing.
• At least 1 year of experience in Nurse or Claim Assessor.
• Excellent in Microsoft Office.
• Basic English communication skill.
